THE INITIAL HEALTH HISTORY CALL

Once a physician’s referral/order or request for services is received by our office, you will be emailed or mailed a link to our online history form to complete. Once this step is completed, someone from our boarding department will reach out to you to clarify anything in question on the form and then schedule your procedure.

You will receive, by mail or email, a package with all the information that you will need to prepare yourself prior to the procedure.

Remember, if you have any questions about your procedure or problems with your preparation, please call the boarding department at (586) 447-0704.

If you would like to schedule an appointment, rather then a procedure follow this link: Schedule Appointment

For all appointments not cancelled or rescheduled at least 24 hours prior to your procedure, there is a $75.00 cancellation/no show fee.

COVERAGE

Our Billing Department can check insurance eligibility; however, you may want to verify your benefits with your insurance company. We suggest contacting your insurance company to obtain coverage information for your procedure or visit to better prepare for possible financial obligations.

SCREENING VS DIAGNOSTIC COVERAGE

Insurance companies often provide coverage for screening colonoscopy. However, if during your screening procedure the physician removes a polyp or performs a tissue biopsy, the procedure may ultimately be considered diagnostic and may no longer qualify for coverage as a screening service. In this case, you may be responsible for all or part of the procedure costs. It is important to research these matters prior to having a procedure.
